A LETTER TO THE INDIAN PUBLIC TO UNDERSTAND THE FLAWS OF THE INDIAN HEALTHCARE SYSTEM






A letter to the Indian public .. 
Being anti doctor is not the treatment for poor healthcare system .
We as well as you need to understand the pathogenesis of it all .
What I understand by looking all around is 
The economic reforms brought about the introduction of corporates in all fields not only medicine .
 Big corporates involved big investments .To get back the profits from their huge investments these business houses required consumers in multitudes who could pay well .
This was in all sectors and healthcare was just one of them . Intermediaries and journalism helped this axis in their own way . All the other sectors catered not to the essential needs but healthcare was part of an essential need .Hence while telecom ,tourism ,engineering business ,entertainment business or be it any other business that catered to the middle class needs which were not essentially required were easily accepted as middle class could afford it even if they took a big pick at the pocket (and they never complained for it ) ; the essential needs like healthcare ,food procured through farmers ,water etc etc became difficult to attain for the poor class because the small players in these essential services were too given the same treatment as the big players ie the corporates .The same rules held ,innumerous taxes levied . The man who was ready to pay Rs 3000 -5000 for a hotel room just for bed was not willing to pay even Rs 2000 for a hospital bed where he was not only being cared for (using the electricity continuously while in hotels he would be there only when he was not outdoors  while he/she was fighting for good health and life )by a doctor who at times would be missing on his meals and social commitments while attending to him  (this i talk about one man run nursing home not corporates where there is a force to look after ) . The taxes imposed on this one man run business were same as on corporates .. The axis of political- corporate -insurer iconoclasts have unintentionally made the life of small clinics and nursing home doctors pathetic and to keep up with the involved expenditures for keeping in pace with these corporates they have to on one hand invest heavily by taking loans and on the other hand the patients who are willing to pay the hotels are not willing to pay for their expertise . This doctor now has to face the brunt of the patients wrath too which actually should be against the politico-corporate -insurer axis . The journalists who are not investigative and report the incidents as such complicate the situation .
Now let us analyse the result of commercialisation of education .On one hand is reservation with its bad effects and on the other hand the management seats and NRI SEATS are accompanied with unbridled donations (under the table ) where there is payment of non documented money for non deserving candidates . Reservation is also resulting at times in deserving candidates having to pay heavy donations to procure seats which they were entitled to but cudn't because some undeserving candidate of reserved category got through even after scoring 2 % marks --- HEIGHT OF IRONY  THIS IS .
This education system of reservation and donations is producing a crop of undeserving doctors ,engineers and providing a weak foundation and infrastructure to our country further weakening it .
When reservation system was brought in it was for a particular duration to equalise the caste divide . Many years have passed beyond that limit and it is still in place ..The reservation seats are now filled up by the affluent of the reserved class .This system needs a looking into and revised so that admissions are not based on reservation and donation and based on purely merit and poor students be they from any caste given scholarships if we really wish to see a strong India .Govt need to understand that reservation should be removed from all competitive exams .If it needs to provide facilities for reserved category it should form separate education institutes for them and give them admissions and facilities there .Please dont play with the future of deserving candidates and the country .
Next point is all the documentation in all govt and private set ups should be e-based so that it is easy to compile data , do studies and form strategies and catch the defaulters .
Laws should be few ,clear well defined and all laws which are dubious and which in past have been interpreted differently in different courts should be seriously looked into and reformulated for a better implementation .
YOU NEED TO UNDERSTAND THAT TO GET A BETTER HEALTHCARE GOING AGAINST DOCTORS WILL NOT HELP THAT WILL FURTHER INCREASE THE DIVIDE AND MAKE DOCTORS CAUTIOUS WITH MORE INVESTIGATIONS INCURRING MORE EXPENDITURE .THE ATTACKS WILL INCREASE DOCTORS TO OPT FOR MEDICAL INSURANCE PROTECTION AND FOR PROTECTION BY PRIVATE ARMED PERSONALS AND WILL ONLY FORCE ADDING UP ON EXPENDITURE ON YOU AND WILL  REFLECT  ON THE TREATMENT CHARGES WHICH WILL FURTHER INCREASE . YOU HAVE TO JOIN HANDS WITH DOCTORS TO STAND AGAINST THE ONSLAUGHT OF THESE BIG GOVT-CORPORATE- INSURER AXIS . SMALL NURSING HOMES ARE VERY MUCH NEEDED TO COUNTER THE CORPORATE-INSURER AXIS . FOR BETTER HEALTHCARE FIGHT WITH DOCTORS FOR THE FOLLOWING CHANGES TO BE BROUGHT ABOUT IN THE SYSTEM --
1- Doing away with donations and reservations in medical entrance examination .
2- To do away with management and NRI SEATS in private medical colleges ..Though increased fees in these institutions is justified .
3 .To treat health care services as essential services and to keep them free from commercial taxes .
4 . To allow small clinics and nursing homes to run in residential areas .
5 . To implement the act which is in place to Stop Violence Against Doctors .

Commercialisation is seen in every  action of ours  as citizens, as part of our lives.
Most of you who deliver in private hospitals also prefer to go to private schools which are just a shade better than the "aided" ones and while the fees there are about 50-60,000 per year, the same you who gladly pays these astronomical fees, is NOT HAPPY parting with a few thousands for a normal delivery or for that matter an LSCS which is concerned with your life .

The crass commercialisation brought in by coaching institutes is another aspect which drains an Indian couple of the money. Add to this the "Mall culture" where people "tend" to spend thousands on a movie ticket, and on personal wear but "Hate" to spend it on self care.

You wouldn't mind spending thousands on a beauty treatment, but worry when you fall ill .

The mentality that "Healthcare should be free " is the major upsetting factor. 

You do not realise that it's wise to get "Health Insurance" , invest in long term bonds, etc to reap benefits of money saved in a trickle to yield good returns at the "required time"

Dr Parag agrees with my point on breaking the "unholy nexus" of "Govt-Corporate-Insurer" nexus . It throws up many questions on the "policies" of the govt which appear like a "sugar coated pill"

He has further analysed well -- Most corporate hospitals have acquired land from govt, and some even pledge few beds for the "poor" section , but these beds are never available to the "needy" They are filled by the management recommended patients.

As compared to the "professional" care given by corporate where, 
1) doctor sees patients just once a day,
2) Staff keep pain scores more than do TPR and monitoring
3) where close monitoring NEVER happens in ward.
4) Patient is put in ICCU for post OP care just because intensive monitoring in post op is NOT possible!!!!!!
5) Doctors get just 10% of the bill amount and has 100% liability!!!!!
6) Major profit making person who delivers healthcare is "kept at the least involvement in the care......and given peanuts...

Major portion of the money spend goes to "maintenance costs" and These corporate hospitals aren't as swank as compared to the money they ask for................

The private nursing home has more "homely atmosphere"
1. Doctors see the patient many times a day
2. Close intensive monitoring is done in what ever room choice patient makes
3. Doctors get good 30-40-60% of the hospital charges
4. Patients BENEFIT the most as the total spend is about just 30-40-50% of a corporate bill, depending on the case.
IN OBGYN , delivery cases the patient just pays few thousands as compared to lakhs in the corporate.
HENCE I ASK YOU THE PUBLIC TO UNDERSTAND THESE POINTS AND   STAND WITH DOCTORS AND RAISE VOICE WITH THEM FOR A BETTER HEALTHCARE .DON'T LET THE INTERMEDIARIES ENTER HEALTHCARE SYSTEM INCREASING THE NUMBER OF POCKETS YOU HAVE TO PAY FOR AND CASH ON YOUR HEALTH . STAND WITH DOCTORS TO BRING IN CHANGES IN THE HEALTHCARE SYSTEM .
